Do I need a box spring with this bed?

No. The frame has slats built in, so a mattress sits directly on them. That saves the cost of a box spring.

What mattress thickness works with it?

A standard queen mattress works. A mattress around 8 to 12 inches thick keeps the wingback headboard visible behind it.

The frame is rated for up to 800 lb. That covers most couples plus bedding.

Not especially. Vacuum it with a brush attachment and spot clean stains quickly, since upholstered fabric holds marks.
